Fascial Manipulation

Fascial Manipulation is a manual therapy method that has been developed by Luigi Stecco, a physiotherapist from Italy. This method has evolved over the last 40 years through study and practice in the treatment of Musculoskeletal Pain and dysfunction.

It focuses on the fascia, in particular the deep muscular fascia, including the epimysium and the retinacula and considers that the myofascial system is a three-dimensional continuum. Treating the Fascial system helps in restore muscular balance and reduce stiffness. The goal of fascial manipulation is to restore gliding along fascial layers and normal activation of myofascial lines of tension to restore balance within neuromusculoskeletal system.

Why fascia is important?

The body has 14 segments and that each body segment is essentially served by six myofascial units (mf units) consisting of monoarticular and biarticular unidirectional muscle fibres, their deep fascia (including epimysium) and the articulation that they move in one direction on one plane. Numerous muscle fibres originate from the fascia itself and, in turn, myofascial insertions extend between different muscle groups to form myofascial sequences.

In some people, the deep fascia densifies or hardens which can restricts the fascial gliding there by altering the movement pattern. This can cause pain and dysfunction in the musculoskeletal system. By releasing the fascia at certain coordination (CC) and fusion points (CF) of the fascia, the normal gliding of the fascia is restored. This leads to normalization of movements and reduction of pain.

How the treatment is given?

The patient is evaluated based on history, pain locality, painful movement and restriction. There by the specific points in specific fascial chains are identified. A strong deep pressure is applied over those points to a point of creating a local inflammation.

What Conditions treated by Fascial Manipulation

Repetitive Stress injury, muscle tendonitis, neck pain, headache, rotator cuff impingement ,frozen shoulder, carpal tunnel syndrome, sacroiliac joint dysfunction, sciatica, muscle strains, iliotibial band syndrome, patellofemoral dysfunction, plantar fasciitis. In all muscle and joint related pain, the fascia would be involved and contributing to the pain.

What can expect during and after treatment ?

Most people experience local soreness and muscle pain lasting 2-12 hrs after treatment. In some cases, the local inflammation can cause pain up to 2 days depending upon the nature and duration of the symptom. The soreness then go away taking away the original pain.

How many sessions might be needed for a patient ?

Four to five treatment sessions are needed for successful treatment. However, if the patient’s condition is chronic in nature with multiple joint involvement and global pain, then it might need more number of sessions. If the diagnosis is correctly made and the treatment points are accurate, the results are often quick.

In our centre, the physiotherapist had obtained a Diploma in Fascial Manipulation and he has completed all three levels of fascial manipulation. He is quite experienced in this technique.
